Individual commuters interested in forming vanpools (see How to Get Started) should only complete Section I of the Commuter Vanpool Application form.

Companies that are forming corporate vanpools and assuming responsibility for monthly vanpool payments (see Corporate) should complete Section II, "Corporate Information," of the Commuter Vanpool Application form.
